4. ORGANIZATIONAL CHART AND FINANCIAL STATEMENTS
ALL DOCUMENTS, PHOTOGRAPHS AND DECLARATIONS MUST NOT BE OLDER THAN SIX (6) MONTHS. THE COPIES OF THE DOCUMENTS TO BE SUBMITTED AS COPIES SHALL BE APPROVED BY THE COMPETENT AUTHORITIES OR THE ORIGINAL SHALL BE SUBMITTED SEPARATELY DURING DOCUMENT SUBMISSION.
PROFESSIONAL EXPERIENCE MUST BE PROVED WITH OFFICIAL DOCUMENTS SUCH AS TRADE REGISTRY RECORD, DOCUMENT TO BE OBTAINED FROM SGK.
INSURANCE AGENCIES MUST MAKE AN AGENCY CONTRACT WITH AT LEAST ONE INSURANCE COMPANY WITHIN 6 MONTHS AFTER REGISTRATION AND SUBMIT IT TO THE CHAMBER WHERE THE APPLICATION IS MADE.
